I am hereby voluntarily consent and agree to the collection, use, disclosure and processing of my Personal Information and Sensitive Personal Information by SUPERCARE MEDICAL SERVICES, INC., as defined by the DATA PRIVACY ACT OF 2012. I acknowledge that my Personal Information and Sensitive Personal Information may/will/can be disclosed by SUPERCARE MEDICAL SERVICES, INC. to parties, such as, but not limited to, medical personnel and specialists, principals, manning agencies/employers, P&I Clubs, and random inspection/audits by regulatory agencies, certification bodies and the likes, or to any third party who may/will/can be involved in the processing of my Personal Information and Sensitive Personal Information for the purpose of my medical examinations, fitness certification, and/or employment/deployment purposes.

What is personal data? Personal data is a collective term referring to all types of personal information. It may be: Personal information, or those which can be used to identify you. This may include your name, address, telephone number, etc. Sensitive personal information are those that may be related to any of the following: racial or ethnic origin, age, marital status, political opinions, religious beliefs, health, sexuality or sex life, offences and/or convictions, government-issued numbers and/or licenses.

What is “processing” of my personal data? “Processing” refers to any operation performed on personal data, including collection, recording, organization, storage, updating, modification, retrieval, consultation, use, consolidation, blocking, erasure, or destruction.

How will you use my personal data? Your personal data will primarily be used for processing of your pre-employment medical examination, application for employment, eventual employment.

Will you share my personal and sensitive personal information? Who will you share it with? We will only share your Personal Information and Sensitive Personal Information with parties who shall be involved in the processing of your medical examinations, fitness certification and your eventual employment/deployment, such as but not limited to medical personnel and specialists, principals, manning agencies/employers, P&I clubs, and random inspection/audits by regulatory agencies, certification bodies and the likes.

Will you share my personal and sensitive personal information online? Yes, however, online medical results shall only be accessible to authorized medical personnel and specialists, principal and your manning agency/employer.

Where will my personal data be stored? Your personal data will be stored securely in the premises of the Medical Clinic.

How long with you store my personal data? All non-electronic copies of your personal data shall be destroyed after five (5) years, counting from your last pre-employment medical examination at the Medical Clinic, by shredding of documents, while all the electronic copies of personal, sensitive personal and privileged information shall be destroyed after thirty (30) years, counting from the your last pre-employment medical examination at the Medical Clinic, by deletion from physical servers and restricted folders.

Can I withhold my consent? Yes, but we may not be able to properly assist you in your pre-employment medical examination.

What if the data is inaccurate? You may dispute the inaccuracy or error and we will correct it accordingly.

What is a Personal Information Controller? A Personal Information Controller is someone who is responsible for your data and who must make sure that your data is processed according to the law.
